SESSION DESCRIPTION:

NJAFP has been working with primary care practices using quality improvement methods to implement measurable, sustainable change in immunization rates. A key aspect of this program has been to leverage the NJDOH Immunization Standards and Job Aids for practices to self-assess gaps and opportunities. This session will cover the Job Aids and how they were used, strategies used by participating practices to improve vaccination rates and the learning and results to date.

LEARNING OBJECTIVES:

  • Use the NJDOH Immunization Standards and Job Aids to self-assess vaccination processes in their own practice
  • Describe learning from participating practices on methods to improve vaccination rates
  • Test change ideas in their own practice to improve vaccination rates
